I/we certify that all information provided in connection with this request is true, complete, and correct to the best of my/our knowledge. I/we understand that Connected Capital Funding, LLC and/or its assigns, lenders, agents, investors, representatives, and affiliates may rely on this information in evaluating this request.

I/we authorize Connected Capital Funding, LLC and/or its assigns, lenders, agents, investors, representatives, and affiliates to obtain and review consumer credit reports, background reports, public records, employment information, banking information, asset information, rental or mortgage payment histories, and any other information reasonably necessary to evaluate, underwrite, service, or collect on this request and any related obligation.

I/we understand that this authorization may be used to verify information now and from time to time in the future in connection with any review, underwriting, renewal, extension, modification, servicing, enforcement, or collection activity related to this request or any resulting loan.

I/we authorize any creditor, financial institution, employer, landlord, mortgage servicer, title company, government agency, or other person or entity having information about me/us or my/our accounts to release such information to Connected Capital Funding, LLC and/or its assigns, lenders, agents, investors, representatives, and affiliates upon request.

I/we acknowledge that a photocopy, electronic copy, facsimile, or other reproduction of this authorization shall be as valid as the original.

Planet Management Group, LLC dba Planet Capital

Credit & Background Authorization

To whom it may concern: I hereby authorize Planet Management Group, LLC dba Planet Capital ("PRC") to conduct: (1) a consumer credit report and verify other credit information, including past and present mortgage and landlord references; and (2) a background investigation report and verify both criminal and civil records.

It is understood that a copy of this form will also serve as authorization by me allowing PRC to conduct these checks.

The information PRC obtains is only to be used in conjunction my inquiry into the commercial mortgage I am considering.

This authorization expires 30 days from the date below.

Privacy Act Notice: This information is to be used by the agency collecting it or its assignees in determining whether you qualify as a prospective mortgagor under its program. It will not be disclosed outside the agency except as required and permitted by law.

You do not have to provide this information, but if you do not your application for approval as a prospective mortgagor or borrower may be delayed or rejected.
